2010 ANNUAL SURVEY REPORT

The Annual Survey covering the 2009 figures reported by 74 hospices in the State of Wisconsin is attached.  We appreciate the cooperation of those hospices who responded to this survey and extend sincere thanks to all hospice staff who took the time and effort to provide information about their services and patients.

Each individual hospice survey report is also attached, alphabetically by hospice name.

2009 ANNUAL SURVEY REPORT

The State 2009 Annual Survey covering the 2007 figures has been completed and is now available.  Because of its size, it was necessary to break it up into four sections:
