Hospice - Veterans Partnership of Wisconsin and Northern Michigan

Steering Committee Statement of Purpose

         

Purpose:  The purpose of the Hospice - Veterans Partnership of Wisconsin and Northern Michigan Steering Committee is to form a state-wide organization that will provide leadership, technical assistance and program development recommendations aimed at: 1) strengthening the relationships between community hospice agencies and VA facilities and 2) improving veterans' access to hospice and palliative care across all sites and levels of care.


Objectives

  1. Convene a statewide Steering Committee to guide the formation of HVP of Wisconsin and Northern Michigan.
  2. Assess the activities occurring in Wisconsin and Northern Michigan related to the provision of hospice care to veterans
  3. Develop a plan to educate partners and include existing coalitions at the community level to address the mission
  4. Plan statewide educational event(s) to provide information on hospice and the VA healthcare system to Wisconsin and Northern Michigan hospices and VA facilities

Membership:  The HVP of Wisconsin and Northern Michigan Steering Committee will be comprised of about 15 members representing the following groups of stakeholders:

  1. State Hospice Organization
  2. Community hospice agencies
  3. VA facilities - VISN, VA Medical Centers, Community-Based Outpatient Clinics (CBOC)
  4. State Veterans Homes
  5. County Veterans Service Organizations (CVSO)
  6. Veteran and family member
  7. Other community, state and national organizations interested in improving care through the end-of-life for veterans

Initially, the HVP of Wisconsin and Northern Michigan Steering Committee will appoint co-chairs representing community hospice and VA facilities to provide leadership and guidance in the formation of the partnership.  Once the partnership has been formed, members will determine how its leadership is selected and define specific responsibilities.


Responsibilities:  The HVP of Wisconsin and Northern Michigan Steering Committee will convene primarily by conference calls each month between quarterly face-to-face meetings of the general membership.  Members will be expected to contribute their time, expertise, and energies as appropriate in meeting the purpose of the coalition, including, but not limited to:

  • Representing the interests of their stakeholders group
  • Participating in the regular meetings
  • Building partnerships
  • Seeking funding to support HVP activities
  • Conducting state-wide and regional educational events
  • Making recommendations regarding improving hospice and palliative care services for veterans in Wisconsin and Northern Michigan

All veterans have quality hospice and palliative care at the time and place of need.


Mission

The mission of HVP of Wisconsin and Northern Michigan is to establish an enduring network of hospice and VA professionals, volunteers and other interested parties working together to provide quality services through the end-of-life for all Wisconsin and Northern Michigan veterans.
